The story follows (Bautista), a veteran elite assassin who is diagnosed with a terminal neurological disease. To avoid a slow and painful death, and to ensure his girlfriend Maize (Sofia Boutella) receives a life insurance payout, he takes out a contract on his own life. Shortly after, Joe discovers he was misdiagnosed due to a medical lab error. However, the contract cannot be canceled, forcing Joe to fend off a quirky army of international assassins—some of whom were former colleagues—to save himself and protect Maize.
